Eligibility

Inclusion criteria

Inclusion criteria: Patients classified as American Society of Anaesthesiologists (ASA) physical status I, II OR III.

Exclusion criteria

Exclusion criteria: 1. Patient refusal to participate or undergo regional anesthesia. 2.Known allergy or hypersensitivity to local anesthetics or study drugs (Bupivacaine, dexamethasone, ketamine, dexmedetomidine,lignocaine). 3.Infection at the block site or local skin pathology. 4. Coagulopathy or bleeding disorders 5.History of severe cardiovascular or respiratory instability contraindicating sedation or regional anesthesia. 6. Neurological disorders affecting pain perception or sensory mapping. 7. Patients on chronic opioid or sedative use, which may influence block efficacy. 8.Inability to understand or communicate pain scores reliably (e.g., severe cognitive impairment). 9.BMI more than 35 kg per m , as excessive adiposity may interfere with block performance or ultrasound visualization

MeasureTime frame
To assess the success rate of modified radical mastectomy surgeries completed under ultrasound guided PECS block with sedation without conversion to general anaesthesia. To assess intraoperative haemodynamic stability and to document complications or adverse events related to PECS block and sedation.Timepoint: Before block, just after block ,then every 5 min. till 30 mins and then 1 hrly till the end of surgery

To assess postoperative pain scores using the Numerical Rating Scale To assess total analgesic consumption during the first 24 hours after surgeryTimepoint: To observe postoperative pain scores using NRS at 30mins 1,2,4,6,8,10,12,16,20 and 24 hours
