Eligibility

Inclusion criteria

Inclusion criteria: Geriatric patients receiving more than or equal to 5 drugs

Exclusion criteria

Exclusion criteria: 1. Critically ill patients, as assessed by the treating physician at the time of recruitment 2. Patients with acute delirium or severe cognitive impairment, as determined by the treating physician, that would interfere with study participation 3. Patients taking medications for infectious diseases at the time of recruitment 4. Currently enrolled in other interventional medication trial

To compare the effect of FORTA-guided and Beers criteria-guided drug reconciliation on health-related quality of life (measured by IGQOLI) in geriatric patients.Timepoint: At baseline, 1 month and 3 months of follow up

To compare the effectiveness of FORTA list-guided drug reconciliation versus Beers criteria-guided drug reconciliation in reducing potentially inappropriate medications (PIMs) in geriatric patients.Timepoint: 1 month , 3 months;To evaluate & compare the acceptance rate of structured reconciliation recommendations between FORTA-guided & Beers criteria-guided interventionsTimepoint: 3 months;To evaluate Adverse eventsTimepoint: 3 months;To evaluate clinically significant drug-drug interactionsTimepoint: Baseline, 1 month, 3 months;To determine the proportion of medication changes made for reasons not addressed by FORTA or Beers criteriaTimepoint: 3 months
