Eligibility

Inclusion criteria

Inclusion criteria: 1.Adults aged 60 years and more. 2.Elective non cardiac surgery. 3.Transthoracic echocardiogram ordered as part of routine preoperative assessment (not for acute indications). 4.Surgery performed or medically cancelled within 60 to 90 days of the index echocardiogram, with documentation linking decision making to the preoperative episode. 5.Availability of essential preoperative clinical data required for model derivation (age, sex, surgical category, cardiac history, symptoms, functional capacity, ECG)

Exclusion criteria

Exclusion criteria: 1.Emergency or urgent surgery. 2.Cardiac or major aortic surgery. 3.Echocardiogram ordered for acute or urgent indications, including 3.1.Acute chest pain or suspected Acute Coronary Syndrome (ACS) 3.2.Acute decompensated heart failure 3.3.Suspected endocarditis 3.4.New arrhythmia needing immediate evaluation 8.Surgery cancelled for non medical reasons, including administrative, logistical, or patient-preference reasons

Design outcomes

Primary

MeasureTime frame
To derive and internally validate a multivariable prediction model that uses only routinely available preoperative clinical variables, measured before requesting a transthoracic echocardiogram, to estimate the probability that the echocardiogram will lead to an actionable change in perioperative cardiac management in adults aged 60 years and above undergoing elective non-cardiac surgeryTimepoint: The day before the surgery date

Secondary

MeasureTime frame
Perioperative MACE (MI, cardiac arrest, new arrhythmia requiring treatment, acute heart failure, stroke) Timepoint: 24 hours, 7 day & 30 day;Surgical delay (days from echo order to surgery date) Timepoint: On the day of surgery;30-day mortalityTimepoint: 30 days following surgery
