Eligibility
Inclusion criteria
Inclusion criteria: - Adults aged 18 years and above - Diagnosed with hypertension - Registered at the selected HWCs
Exclusion criteria
Exclusion criteria: - Individuals lacking basic proficiency in using an Android mobile phone will not be eligible to participate in the study - Patients with severe cognitive impairment or unable to provide informed consent - Patients with severe illness or hospitalization at the time of the study
