Eligibility

Inclusion criteria

Inclusion criteria: Diagnosed head injury requiring Neuro ICU On invasive mechanical ventilation via tracheostomy tube for more than or equal to 48 hours Clinical suspicion of lower respiratory tract infection

Exclusion criteria

Exclusion criteria: Hemodynamic instability or recent cardiac arrest where the procedure is unsafe Severe hypoxemia or severe bronchospasm at the time of planned sampling Uncorrected coagulopathy or platelet count less than 50000 Upper airway anatomical abnormalities preventing safe catheter advancement Respiratory specimen already collected within the previous 24 hours for clinical purposes Severe immunosuppression where additional procedures are deemed high risk
