Eligibility
Inclusion criteria
Inclusion criteria: Survivors above 18years who are getting discharged from the hospital with an ICU admission of 48 hours will be included. Informal caregivers staying with them at least for a month or most of the time in a month, having a smartphone and who can understand English, Kannada, and Hindi will be included.
Exclusion criteria
Exclusion criteria: Survivors with pre-existing disabilities, those admitted following any surgeries, those with multiple prior ICU admissions, or those discharged with palliative intent will be excluded. Caregivers with difficulty in comprehending or those who will not be present after patient discharge will be excluded.
