Inclusion criteria

Inclusion criteria: The study will be carried out in multiple geographical locations near to MRHRU Sagalee of Arunachal Pradesh covering all inhabitants aged 18 years and above. Inclusion criteria: Adults aged 18 years and above - Diagnosed with hypertension - Registered at the selected HWCs

Exclusion criteria

Exclusion criteria: Exclusion criteria: - Individuals lacking basic proficiency in using an Android mobile phone will not be eligible to participate in the study - Patients with severe cognitive impairment or unable to provide informed consent - Patients with severe illness or hospitalization at the time of the study - Those who do not give consent will be excluded from the study

To assess hypertension care cascade in a selected catchment area under Health and wellness centers of Arunachal Pradesh. Capacity building of frontline health workers of Health and Wellness Center in usage of a unique android based mobile application for adopting healthy lifestyle practices and self-monitoring of blood pressure for hypertension management. Community empowerment for use of a unique android based mobile application (mHealth) through front line health workers of Health and Wellness Center.
