Inclusion criteria
Inclusion criteria: Group 1: individuals residing in the vulnerable area and who are aged 30+ years neither with Diabetes or Hypertension. Group 2: individuals residing in the vulnerable area and who are aged 30+ with Diabetes or Hypertension Qualitative exploration will be conducted with the frontline workers, facility staff specially allied health staff, community structure
Exclusion criteria
Exclusion criteria: Group 1 : A person with is not able to participate due to severe morbidity or disability or adults aged below 30 and unwilling to consent Group 2 : A person with is not able to participate due to severe morbidity or disability or adults aged below 30 and unwilling to consent

| Group 1 The study will assess barriers to screening for diabetes & hypertension including awareness accessibility & utilization of UPHC services. It will evaluate patterns of health service utilization & levels of community engagement in NCD prevention. Feasibility of screening services at the community level will also be assessed. Group 2 The study will assess barriers to treatment adherence & lifestyle modification including access to care & followup services. It will evaluate health service utilization for ongoing management of diabetes & hypertension. Community engagement & the feasibility & acceptability of service delivery for NCD care will also be explored.Timepoint: 18 months | — |
