Eligibility

Inclusion criteria

Inclusion criteria: 1) Adult patients greater than 18 years of age with Type 2 Diabetes Mellitus 2) Enrolled in Madhavbaug CDC program between April 2019 and July 2025 3) Completed 90-day CDC program 4) Valid paired HbA1c measurements, both at baseline and at Day 90 5) HbA1c values within physiologically valid range 4 to 15
