Inclusion criteria

Inclusion criteria: 1 Elderly patients more than 60 years on mechanical ventilation for more than 48 hours 2 Eligible for SBT

Exclusion criteria

Exclusion criteria: 1 Short term mechanical ventilation less than 48 hours 2 Spinal cord injuries above T8 3 Diaphragmatic paralysis or neuromuscular weakness 4 Poor ultrasound windows 5 Encephalopathy 6 Advanced Heart failure severe valvular heart disease persistent arrhythmias affecting Doppler measurement
