Inclusion criteria: All medications and procedures were administered to adult patients in OT and AICU during the study period.
Exclusion criteria
Exclusion criteria: Pediatric Patients and TPN patients

| Primary Outcome- To determine the nature and frequency of medication errors while prescribing, preparing, storing the prepared medication or while administering the drugs to the patients in OT and AICU. Timepoint: Data will be collected at the time of drug prescription, preparation and administration during intraoperative period and ICU stay until discharge or transfer, which is 1year 6 months of trial period | — |

| To study the extent of the medication error in terms of whether the error reached the patient or not. To study the harmful effects of medication errors on the patient. To study the communication of the medication error to the patient or the caregivers.
