Eligibility

Inclusion criteria

Inclusion criteria: 1.Adults more than 18 years scheduled for elective Off pump Coronary artery bypass Grafting. 2.Written informed consent

Exclusion criteria

Exclusion criteria: 1.Emergency or redo CABG. 2.Preoperative hemodynamic instability requiring inotropes or mechanical support ( intra aortic balloon pump,extracorporal membrane oxygenation) 3.Significant valvular disease (moderate or severe mr/tr/as/ar) or other concomitant cardiac procedures. 4.Persistent atrial fibrillation or arrhythmia preventing reliable annular excursion measurement. 5.Contraindication to tee (esophageal pathology, bleeding diathesis, recent upper GI surgery). 6.Poor TEE image quality despite probe optimization.

MeasureTime frame
1.Incidence of intraoperative conversion from off-pump coronary artery bypass (OPCAB) to on-pump coronary artery bypass (ONCAB) in relation to post induction echocardiographic indices (TAPSE) measured using transesophageal echocardiography (TEE). 2.To determine whether post induction TAPSE values can serve as independent predictors of the need for conversion from OPCAB to ONCAB. Timepoint: Intra-operative (from the start of coronary artery grafting until the completion of the procedure).

MeasureTime frame
1. To determine whether post induction MAPSE values can serve as independent predictors of the need for conversion from OPCAB to ONCAB. 2. Evaluation of combined predictive accuracy (TAPSE + MAPSE) for intraoperative conversion, using ROC curve analysis to determine optimal cutoff values. 3. Postoperative outcomes including duration of mechanical ventilation, mean vasoactive-inotropic score over 24 hours, and length of ICU stay. Timepoint: Intra-operative (from the start of coronary artery grafting until the completion of the procedure).
