Intervention1: High pressure Bipap: Ipap of 20-30 cm H2O till pH reverts to normal and hypercarbia resolves and the patient can be safely weaned off Bipap therapy. 3-6 days. Control Intervention1: Low

Eligibility

Inclusion criteria

Inclusion criteria: Age more than 18, previously known or probable case of COPD, ABG showing acute type 2 Respiratory failure

Exclusion criteria

Exclusion criteria: Need for immediate intubation, contraindications to NIV, hemodynamic instability, poor sensorium, or facial trauma, overt clinical features of Heart failure, Age less than 18 years

Acidosis reversal within 24 hours with decrease in PCO2Timepoint: Acidosis reversal within 24 hours with decrease in PCO2

MeasureTime frame
Duration of hospital stay. Delaying the need for intubation, Patient tolerance to high pressure NIV, HACOR and ROX score improvementTimepoint: 0,1,6,12 and 24 hours and throughout the hospital stay
