Eligibility
Inclusion criteria
Inclusion criteria: Possess a smartphone or have access through a family member. Able to read and understand Hindi and English. Willing to participate and provide written informed consent
Exclusion criteria
Exclusion criteria: Patients with cognitive impairment or inability to use smartphone. Patients having Terminal illness more than3 months expected survival. Patients with permanent communication disability. Patients already enrolled in other self-care trials or using similar apps.
