Eligibility

Inclusion criteria

Inclusion criteria: 1. Age 18-70 years 2. Either sex 3. Scheduled for valvular surgery under cardiopulmonary bypass, including Aortic valve replacement, Mitral valve replacement/repair with tricuspid valve repair or Double valve replacement

Exclusion criteria

Exclusion criteria: 1. Not willing to participate 2. Emergency surgery 3. Prior cardiac surgery/ Redo surgery 4. Functional hepatic disease 5. Functional renal disease 6. Pre-operative acute kidney injury 7. Severe pre-operative renal dysfunction; eGFR 8. Kidney transplant recipients 9. Patients receiving peri-operative mechanical circulatory support (IABP or ECMO) 10. Pre-operative delirium 11. Any congenital/acquired cerebrovascular/neurological disorder 12. History of psychotic drugs intake 13. Alcohol and substance abuse 14. Pregnancy

MeasureTime frame
1.To determine the sensitivity and specificity between VExUS score grades and the incidence and severity of acute kidney injury (AKI) using KDIGO criteria within 48 hours of valvular surgery. 2. To determine the sensitivity and specificity between VExUS scores and the incidence and severity of post-operative delirium, using the CAM-ICU 7 assessment tool within 48 hours of valvular surgery.Timepoint: Till 48 hours after the cardiac surgery

1. To determine the sensitivity, specificity, predictive value of each ultrasound assessment (Hepatic, Portal and Intrarenal veins) of the VExUS score for AKI and delirium. 2. To determine the sensitivity, specificity, predictive value and hazards ratio of NT-pro BNP with postoperative AKI and delirium after valvular surgery. 3. To determine the association of combination model of VExUS score and NT pro-BNP for development of AKI and delirium. 4. To identify the association between key risk factors (age, comorbidities, anaesthesia duration, CPB, cross clamp time, mean CVP value) with AKI and postoperative delirium.Timepoint: Till 48 hours after the cardiac surgery
