Eligibility

Inclusion criteria

Inclusion criteria: Age 18 45 years Pregnant female scheduled for c-section under Spinal anaesthesia or General anaesthesia High risk for PPH (e.g uterine atony, multiple gestation, polyhydramnios, macrosomia, prior PPH, Pre- eclampsia and prolonged labor) Hemodynamically stable ASA (American Society of Anesthesiologist) physical status (1-4)

Exclusion criteria

Exclusion criteria: Hemodynamically unstable: Hypotension, Hypovolemia Pre-existing use of alpha-2 agonists Hyposensitivity to Dexmedetomidine 2nd or 3rd degree heart block. Sick sinus syndrome Fetal heart block

MeasureTime frame
To evaluate hemodynamic stability. To compare the need for blood transfusion between groups. To assess maternal morbidity (organ dysfunction, ICU stay, need for surgical intervention). To assess the incidence of ICU admission and duration of hospital stay. To evaluate sedation and pain scores. To evaluate effect of dexmedetomidine on fetus (APGAR score), before surgery FHR and post-surgery FHR. To document any adverse effects attributable to dexmedetomidineTimepoint: After starting Dexmedetomidine infusion Before delivery of Baby After delivery of Baby , 24 hr post delivery.
