Eligibility

Inclusion criteria

Inclusion criteria: 1 Adults 30 years and more with at least one modifiable NCD risk factor eg raised Blood pressure raised random blood sugar obesity or overweight high salt intake inadequate fruit and vegetable intake inadequate sleep smoking alcohol use physical inactivity Stress and depression 2 Permanent residents of the selected urban slum areas or rural areas 3 Willing to provide informed consent and follow up

Exclusion criteria

Exclusion criteria: Individuals with diagnosed NCDs under active treatment. (except Hypertension and Diabetes) Pregnant women. Individuals with severe cognitive impairments limiting participation.
