Eligibility

Inclusion criteria

Inclusion criteria: american society of anesthesiologists ASA physical status I or II Elective non-cardiac , non-neuro surgery under general anesthesia Expected duration of surgery less than 2 hours. Use of vecuronium as neuromuscular blocker Planned reversal with either neostigmine or sugammadex MMSE score more than or equal to 24 (normal preoperative cognitive status) Hemodynamically stable intraoperatively Able to provide informed consent Proficient in the language used for cognitive or delirium assessment Postoperative care planned in PACU followed by ward admission

Exclusion criteria

Exclusion criteria: ASA physical status III or higher Emergency surgeries Known neurological or psychiatric disorders Preoperative cognitive impairment (MMSE less than 24) Allergy or contraindication to neostigmine or sugammadex Recent use of benzodiazepines, anticholinergics, or antipsychotics Significant renal (eGFR less than 30 ml per min per 1.73m ) or hepatic dysfunction Intraoperative hemodynamic instability or unplanned ICU admission Communication barriers (language, severe hearing or vision impairment, aphasia) History of alcohol or substance abuse Participation in another clinical trial within the past 3 months

To evaluate and compare the incidence of postoperative delirium (POD) in geriatric patients (age more than or equal to 60 years) with ASA physical status I or II, undergoing general anesthesia with neuromuscular blockade reversed using neostigmine versus sugammadexTimepoint: at 2 hours, 6 hours, 12 hours , 24 hours and 48 hours postoperatively 2 months

To assess the time to emergence and extubation in both groups following administration of the reversal agentTimepoint: at 2 hours, 6 hours, 12 hours , 24 hours and 48 hours postoperatively 2 months
