Inclusion criteria

Inclusion criteria: Patients attending the Health and Wellness centers in these 3 designated healthcare blocks will be screened as per the following criteria: Inclusion criteria a. Age more than 18 years b. Moderate to high risk of AKI as per the ISN 0by25 clinical assessment tool in the ISN AKI toolkit (risk score more than or equal of 3)
