Eligibility

Inclusion criteria

Inclusion criteria: Patients who are above 18 years old and are diagnosed with CKD stages 4 5 (as per KDIGO staging).

Exclusion criteria

Exclusion criteria: Patients who have received any form of RRT in the past due to AKI or prior kidney transplant. Patients with cognitive impairments or conditions that limit their ability to participate. Patients who cannot read and write.
