Inclusion criteria
Inclusion criteria: Patients Adults above18 yearsDiagnosed with Type 2 diabetes mellitus and or hypertension Able to use a smartphone (self or family member support) Willing to provide informed consent Healthcare Providers Doctors involved in diabetes or hypertension care Willing to use the platform and provide feedback
Exclusion criteria
Exclusion criteria: Patients with gestational diabetes; Patients participating in other clinical study that might confound the results
