Inclusion criteria

Inclusion criteria: 1.Patients diagnosed with any psychiatric disorder . 2.Patients with the age more than 18years. 3.Patients on stable psychiatric medication regimen. 4.Patients willing to provide written informed consent for participation.

Exclusion criteria

Exclusion criteria: 1.Patients who are unable to provide informed consent. 2.Patients with incomplete medical records

Design outcomes

Primary

MeasureTime frame
To assess and determine the incidence of ADRs in psychiatric patientsTimepoint: Baseline (0 day), 4 days, and 8 days from enrolment .Any suspected ADR reported during before discharge will be assessed immediately and recorded.

Secondary

MeasureTime frame
To evaluate the severity & preventability of the reported ADRs.Timepoint: Any suspected ADR reported will be assessed immediately within 1st week & during reporting, causality , severity & preventability will be assessed using standardized scales.(WHO-UMC causality assessment scale ,Hartwigs severity assessment scale & Naranjo algorithm scale ) then the final cumulative analysis of ADR will be done after 6 months
