Eligibility

Inclusion criteria

Inclusion criteria: Community dwelling elderly men and women aged sixty years or above residing in the urban and rural field practice areas of the institute. Participants fulfilling three or more Fried frailty criteria. Participants willing to participate and provide written informed consent.

Exclusion criteria

Exclusion criteria: Elderly with severe cognitive impairment, terminal illness, severe psychiatric illness, acute medical illness requiring hospitalization, or those who are bed ridden and unable to participate in physical activity. Participants unwilling to provide informed consent.

Improvement in the Frailty Index Score in the intervention group compared to control group at 6 months and 12 months. Improvement in the mobility status in the intervention group compared to control group using Short Physical Performance Battery. at 6months and 12 monthsTimepoint: From baseline to End of intervention at day 180 (Six months) and day 365 (twelve months)
