Eligibility

Inclusion criteria

Inclusion criteria: Unilateral full-thickness rotator cuff tear with a documented tear size of 2 to 4 cm (as confirmed by pre-operative MRI or intra- operative findings). Undergone primary, open or arthroscopic, isolated rotator cuff repair (no concurrent procedures like biceps tenodesis or tenotomy, acromioplasty, or distal clavicle excision, unless deemed clinically insignificant and documented). Exactly 6 weeks post-operative after the removal of brace or sling at the time of recruitment and baseline assessment. Defined as an individual actively participating in at least one organized sport or regular recreational physical activity (e.g. gym training, running, swimming, team sports) at least 3 times per week prior to injury, and expressing a desire to return to their pre- injury level of activity. Between 18 and 40 years of age (to minimize confounding factors related to advanced degeneration or growth plate issues). Able to understand and follow rehabilitation instructions, provide informed consent, and commit to the study protocol for the entire duration.

Exclusion criteria

Exclusion criteria: Presence of other significant shoulder pathologies (e.g. glenohumeral osteoarthritis, labral tears requiring repair, recurrent instability, adhesive capsulitis prior to current injury). Significant neurological deficits affecting the upper limb (e.g. cervical radiculopathy, peripheral nerve injury). Systemic inflammatory conditions (e.g. rheumatoid arthritis, lupus). Acute infections or unmanaged chronic diseases that could impede healing or rehabilitation (e.g., uncontrolled diabetes, severe cardiovascular disease). Tear Characteristics: 1. Partial-thickness rotator cuff tears. 2. Massive or retracted rotator cuff tears ( more than 4 cm).

Design outcomes

Primary

MeasureTime frame
Primary outcomes are Pain (VAS), ROM (inclinometer), Functional disability (SPADI). Timepoint: 6 weeks.

Secondary

MeasureTime frame
- Secondary outcomes are Safety, feasibility, comparative effectiveness, and readiness to return to sport. Timepoint: 6 weeks
