Eligibility

Inclusion criteria

Inclusion criteria: Adults aged 18 years and above. Diagnosed with cardiovascular diseases such as ischemic heart disease, heart failure, arrhythmias, valvular disorders, or hypertension. Receiving cardiovascular medication (e.g., antiplatelet, statins, ACE inhibitors/ARBs, beta-blockers, anticoagulants, antiarrhythmic). Hospitalized in cardiology wards or attending cardiology OPD during the study period. Digoxin, Verapamil, Amiodarone, Ntg 2.6, Isosorbide dinitrate, Diltiazem, Metoprolol, Propranolol, Clopidogrel, Apixaban, Rivaroxaban, Dabigatran, Warfarin

Exclusion criteria

Exclusion criteria: Patients with incomplete or missing medication records. Patients unwilling to provide consent (for prospective studies). Very short-stay transfers or discharges (24 hours) where complete prescription data cannot be obtained. Patients enrolled in clinical trials where investigational drugs may confound DDI assessment.

MeasureTime frame
The study is expected to provide a clear estimate of the prevalence and severity of potential drug drug interactions (pDDIs) among cardiovascular patients in a tertiary care setting. It will identify the most common high-risk drug combinations and highlight predictors of pDDIs such as age, comorbidities, polypharmacy, and length of hospital stay.Timepoint: baseline, day 7, and 6 month

MeasureTime frame
The analysis of prescribing patterns will offer insights into the most frequently used cardiovascular drug classes & the extent of polypharmacy in real-world practice. These findings are anticipated to help clinicians recognize high-risk situations, support the development of targeted interventions for safer prescribing, & contribute to the implementation of clinical decision support tools. Timepoint: baseline, day 7, & 6 month
