Inclusion criteria

Inclusion criteria: Patients aged 18 years and above Diagnosed with type 2 diabetes mellitus or hypertension. Prescribed more than 1 medication.

Exclusion criteria

Exclusion criteria: Pregnant/lactating women. Patients on short-term therapy only (e.g., antibiotics for acute infection). Incomplete prescriptions

Mean number of drugs per prescription in diabetic and hypertensive patients.Timepoint: Baseline, Day 0, Day 7, and 6 months

Secondary

MeasureTime frame
Proportion of patients with polypharmacy. Most commonly prescribed drug classes (antidiabetics, antihypertensives, statins, gastroprotective). Frequency & severity of drug drug interactions. Differences in prescribing between OPD & IPD patients Timepoint: Baseline Day 0 Day 7 & 6 months
