Inclusion criteria
Inclusion criteria: All newborns delivered at home and at health facilities weighing less than 2500 gms and their mothers will be included
Exclusion criteria
Exclusion criteria: Newborns weighing less than 1000 gms will be excluded from the study. LBW newborns who are discharged from hospital after 28 days will not qualify for inclusion in the study. Mothers who consent to participate will be included in the study. LBW newborns born with major congenital malformations will be excluded. Mothers who do not consent to participate and those who intend to leave the study area within a month will not be enrolled in the study.

| Proportion of LBW newborns who - were visited by ANMs and ASHAs, were received cKMC at least 8 hours per day until the baby wriggles out, were exclusively breastfed at 42 days of age, gained weight, length and head circumference at days 14 and 42, sought care from appropriate health facility before 42 days of age, required readmission post discharge, developed possible serious bacterial infection (PSBI). Proportion of mothers with- improved knowledge on exclusive breastfeeding, cKMC, recognition of danger signs in LBW newborn, counselled by ASHA and ANM on all components of enhanced HBNC package, satisfactory maternal outcomes (without continued bleeding, no features of puerperal sepsis) after 42 days of delivery.
