Eligibility

Inclusion criteria

Inclusion criteria: Patients undergoing elective head and neck cancer surgery ECOG status: 1 and 2 BMI: 18.5-30 kg/m2 Surgical duration less than 5hrs

Exclusion criteria

Exclusion criteria: Patient with advance cardio-pulmonary compromise (NYHA class 4) Free flap Major blood loss during surgery- more than maximum allowable blood loss (MABL) i.e.65 ml/kg of ideal body weight. Patients receiving medications known to interfere with hormonal response to stress (e.g. Beta blockers, steroids etc) Coexisting conditions likely to delay gastric emptying like morbid obesity, diabetes, hiatus hernia, gastroparesis, previous gastric surgery (for example, gastric bypass), gastroesophageal reflux disease, enteral feeding, drugs which alter the gastric motility, pregnancy, hypothyroidism, chronic kidney disease

Design outcomes

Primary

MeasureTime frame
To compare levels of Serum cortisol at four time points as a measure of perioperative stress in the two group i.e. those drinking carbohydrate containing clear fluid versus those drinking plain waterTimepoint: At four timepoints: 8 PM a day before surgery, 8 AM on the day of surgery, and at 2 hours and 24 hours after surgical incision

Secondary

MeasureTime frame
To compare levels of C-reactive protein (CRP) and neutrophil to lymphocyte ratio (NLR) at four time points as a measure of perioperative stress in the two group i.e. those drinking carbohydrate-containing clear fluid versus those drinking plain waterTimepoint: At four timepoints: 8 PM a day before surgery, 8 AM on the day of surgery, and at 2 hours and 24 hours after surgical incision; To compare inferior vena cava collapsibility index (IVCCI) in between the two groupsTimepoint: 2 hours after ingestion of carbohydrate containing fluid and plain water; To compare gastric residual volume (GRV) measured by USG in between the two groups Timepoint: 2 hours after ingestion of carbohydrate containing fluid and plain water
