Inclusion criteria
Inclusion criteria: Cadaveric Study: Well preserved viscera of stomach and duodenum Endoscopic Study: Adults (age 18 years and above) undergoing index ERCP or SVE on native papilla for pancreaticobiliary pathology. Adults (age 18 years and above) undergoing diagnostic UGIE.
Exclusion criteria
Exclusion criteria: Cadaveric Study: 1.Decomposed viscera of duodenum and stomach. 2.Viscera with evidence of any surgical intervention or evidence of injury. Endoscopic Study (for distance of major papilla to pylorus): 1.Patients with previous papillary manipulation and sphincterotomy or pre-cut Patients with unsuccessful endoscopic intubation of duodenum due to luminal obstruction, benign or malignant 2.Patients with pyloroduodenal ulcer disease that causes anatomical deformity of the pylorus and duodenum, despite successful intubation of second part of duodenum 3.Patients with pancreatic head disease, benign or malignant, that causes deformity or narrowing of the C-loop of duodenum 4.Acute or Chronic Pancreatitis where there is macroscopic alterations (e.g., edema, erythema, nodularity, narrowing, etc.) of the first and second part of the duodenum 5.Previous gastric or duodenal surgeries with or without bypass 6.Tumors of the ampulla or peri-ampullary area 7.Impacted Choledocholith in the ampulla or spontaneous choledocho-duodenal fistula 8.Macroscopically deformed major duodenal papilla due to any cause 9.Major duodenal papilla located in the middle or edge of a duodenal diverticulum (Periampullary diverticulum) 10.Failed identification of major duodenal papilla at SVE 11.Patients with American Anesthesiologic Association (ASA) grade 3 and above, undergoing ERCP, to avoid prolongation of procedure time 12.Patients undergoing ERCP for severe cholangitis, to avoid prolongation of the procedure time 13.Pregnant patients 14.HIV positive patients 15.Uncooperative patients 16.Patient not giving Informed consent Endoscopic Study (for position of papilla on wall): 1.Patients with unsuccessful endoscopic intubation of duodenum due to luminal obstruction, benign or malignant 2.Patients with pyloroduodenal ulcer disease that causes anatomical deformity of the pylorus and duodenum, despite successful intubation of second part of duodenum 3.Patients with pancreatic head disease, benign or malignant, that causes deformity or narrowing of the C-loop of duodenum 4.Acute or Chronic Pancreatitis where there is macroscopic alterations (e.g., edema, erythema, nodularity, narrowing, etc.) of the first and second part of the duodenum 5.Previous gastric or duodenal surgeries with or without bypass 6.Failed identification of major duodenal papilla on UGIE (often in small flushed papilla or presence of Periampullary diverticulum, etc.)
