Inclusion criteria

Inclusion criteria: 1.All patients aged >18 years attending the tertiary care hospital OPD 2.All patients who are willing to participate and providing informed consent.

Exclusion criteria

Exclusion criteria: 1.Patients who are critically ill or mentally challenged 2.Patients having major psychiatric illness, intellectual disability, illiterate person. 3.Patients who are not willing to participate and do not provide informed consent. lost to follow up
