Eligibility

Inclusion criteria

Inclusion criteria: 1. Adults aged 45 years or more 2. Scheduled for unsedated colonoscopy for constipation with red flags (blood in stools, anemia, weight loss, abdominal pain, recent worsening etc) 3. Willing and able to provide informed consent.

Exclusion criteria

Exclusion criteria: 1. History of colorectal surgery. 2. Inflammatory bowel disease, polyposis syndrome, or severe cardiopulmonary disease. 3. Pregnancy or lactation. 4. Psychiatric illness or inability to cooperate during the procedure.

Design outcomes

Primary

MeasureTime frame
To compare the mean pain scores between the SMWE and AI groups using the Visual Analog Scale (VAS).Timepoint: At baseline and at 12 weeks.

Secondary

MeasureTime frame
To compare insertion time, cecal intubation rate, & polyp detection rate between the two groups.Timepoint: At baseline & at 12 weeks.;To evaluate the need for ancillary maneuvers (abdominal pressure, position changes, variable stiffness usage).Timepoint: At baseline & at 12 weeks.;To assess patient willingness to repeat the procedure using the same technique.Timepoint: At baseline & at 12 weeks.
