Intervention1: Ultrasound guided High volume proximal adductor canal block for postoperative analgesia in below knee surgeries: Subarachnoid Block (Spinal Anaesthesia): Drug & Dose: 2.5 ml of 0.5% Hyp
no repeat dosing planned). Peripheral Nerve Block (Hi-PAC ? High Volume Proximal Adductor Canal Block): Drug & Dose: 40 ml of 0.1% Ropivacaine (10 ml deposited around saphenous nerve, 30 ml deposite
expected analgesic effect lasts up to 24 hours postoperatively. Control Intervention1: Sub-arachnoid block for post operative analgesia in below knee surgeries.: Group A (Control arm): Intervention:
provides intraoperative anaesthesia and immediate postoperative pain relief

Eligibility

Inclusion criteria

Inclusion criteria: 1.Patients aged between 18 and 60 years 2.Patients with American Society of Anesthesiologists (ASA) physical status I, II or III 3.Patients scheduled for below knee surgeries

Exclusion criteria

Exclusion criteria: ? Known allergy to local anaesthetics ? Chronic opioid use ? Infection at the site of block ? Inability to comprehend Numerical Rating Scale(NRS) ? Patient refusal

MeasureTime frame
Time to demand of the first rescue analgesic in either of the group in the post operative period.Timepoint: patient should be observed immediately at PACU(ZERO minute),2hour,4hour,6 hour,12 hour,24 hour

Secondary

MeasureTime frame
The comparison of the total rescue analgesic consumption within first 24 hours after below knee surgeries in both groups.Timepoint: patient should be observed for 24 hours at PACU(zero minute),2 hour,4 hour,6 hour,12 hour ,24 hour;Numerical Rating Scales (NRS) scale at the time, when the patient in either of the groups required rescue analgesic for pain relief.Timepoint: Postoperative pain will be measured using NRS pain scores within 24 hours in both the groups, where the score will be determined by asking the patient to rate the pain from 0-10 where a NRS score of 0 will indicate ?no pain? to NRS score of 10 indicating ?worst pain?.
