Eligibility

Inclusion criteria

Inclusion criteria: 1. ESRD (CKD Stage 5) on maintenance hemodialysis more than or equal to 3 months. 2.IDHtn (SBP rise more than or equal to 10 mmHg from pre- to post-dialysis in more than 30 percent of sessions over 4 weeks). 3. Written informed consent.

Exclusion criteria

Exclusion criteria: 1. Intradialytic hypotension. 2. Known allergy to L-Arginine. 3. Participation in another interventional trial within 30 days. Before and during the course of study 4. Active infection, severe hepatic impairment, sepsis, or systemic inflammation. 5. Ischemic heart disease, heart failure, or use of nitrates or PDE-5 inhibitors. 6. Hyperkalemia (potassium more than 5.5 mEq per Litre).
