Inclusion criteria

Inclusion criteria: 1) those who received atleast 7 benzthanie penicillin injection prior 2) penicillin allergy test negative.

Exclusion criteria: 1) penicillin allergy test postive

Comparison of the effectiveness of pre -injection manual pressure vs benzathine penicillin G buffered with lignocaine in reducing pain score (using Wong Baker Faces pain rating scale) among children with RHD receiving penicillin prophylaxis. Timepoint: Immediately,2 minutes,1 hour ,6 hour

To compare the effectiveness of the two interventions in reducing fear related to intramuscular injection among rheumatic heart disease patients (using Severity Measure for Specific Phobia -Children) To assess the effect of both interventions on physiological response (Heart rate, Blood pressure, spO2) before & after the injection.
