Inclusion criteria

Inclusion criteria: Primary care physicians practicing in public and private sectors in the identified blocks of the district. Providing care for diabetes and hypertension cases Willing to give informed consent to participate in study.

Exclusion criteria

Exclusion criteria: Primary care physicians not providing care to diabetes and hypertension cases Primary care physicians providing care to diabetes and hypertension cases , but not willing to participate in study Practitioners who are likely to move out of the identified study sites for more than one month during study period.

Primary

MeasureTime frame
The primary outcome measure is a reduction of blood pressure and fasting blood sugar at 12-month follow-up with respect to the baseline measurement among the patients treated by primary care physicians.Timepoint: Every 3 months for 1 year

Secondary

MeasureTime frame
Secondary outcome measures at 12-month follow-up, including rates of smoking cessation, alcohol cessation, change in body mass index compared to baseline, change in fruit and vegetable consumption, change in physical activity among patients and prescription of statin to those it is indicated for primary care physicianTimepoint: Every 3 months for 1 year
