Inclusion criteria
Inclusion criteria: Patients discharged on specific anti-diabetic medications: Group 1: Insulin therapy Group 2: Sulfonylurea OHAs Group 3: Non-sulfonylurea OHAs OPD and IPD or OPD/IPD patients
Exclusion criteria
Exclusion criteria: Patients with psychiatric disorders Patients with dementia Pregnant patients Patients with short life expectancy Patients with far away location LAMA (Leave Against Medical Advice) Patients without access to telephone
