Inclusion criteria

Inclusion criteria: Adult patients who will be admitted in the ICU and are mechanically ventilated.

Exclusion criteria

Exclusion criteria: Patients NOK or their legal representative who refuse to participate.

To replace GCS with FOUR score in SOFA scoring system keeping the validity of SOFA in predicting mortality in critically ill patients.Timepoint: At the time of Admission, Daily thereafter 24hrly till discharge

MeasureTime frame
To compare SOFA score with other neurological assessment scales a. Alert,Voice,Pain,Unresponsive (AVPU) b. Richmond Agitation sedation scale (RASS) c. Alert,Confusion, Drowsy,Unconscious (ACDU) d. Motor component of GCS (mGCS) e. Simplified Motor Score (SMS) Timepoint: At the time of admission, daily 24hrly thereafter till discharge
