study to find out whether repeated checks of pupil reactions, optic nerve size, and white blood cell levels can help predict brain damage after a brain aneurysm rupture

Study of the correlation between serial measurements of Neurological Pupil Index (NPi), Optic Nerve sheath diameter (ONSD), WBC count and occurrence of delayed cerebral ischemia (DCI) in aneurysmal Sub Arachnoid Haemorrhage-A prospective observational study. - NIL

Eligibility

Inclusion criteria

Inclusion criteria: 1.Adult patients admitted and undergoing neurosurgical or interventional radiological procedures at Sree Chitra Tirunal Institute for Medical Sciences and Technology for aneurysmal sub arachinoid hemorrhage. 2.ASA (American Society of Anesthesiologists) class 1 2 and 3 3.SAH Hunt and Hess grades of I,II,III and IV 4.Aneurysm of anterior circulation.

Exclusion criteria

Exclusion criteria: 1.Patient refusal or refusal by patient bystanders 2.Patients with previous SAH 3.Absence of feasibility of performing NPi and ONSD assessment on the eye due to either pupillary disease, oculomotor nerve damage ,disease or significant periorbital oedema where pupillary assessment is not possible. 4.History of previous neurosurgery or endovascular neurological procedures 5.History of continued antiinflammatory drug use in the previous 6 months 6.Acute respiratory failure (pneumonia, acute lung injury, ARDS) 7.Progressive neuromuscular disease 8.Pregnant and lactating mothers 9.Heart failure or severe heart disease. 10.Posterior circulation aneurysms

Design outcomes

Primary

MeasureTime frame
1.To use serial NPi for early detection of delayed cerebral ischemia in patients of aneurysmal sub arachnoid hemorrhage using modified rankin scoreTimepoint: at discharge and 3 months after discharge

Secondary

MeasureTime frame
1.To assess correlation between serial NPi and incidence of vasospasm. 2.To examine the correlation between NPi and ONSD to already established prognostic indices like CT criteria, WBC count, and TCD in the determination of DCI. 3.Correlation between NPi and neurological outcome Timepoint: 21 months
