Eligibility

Inclusion criteria

Inclusion criteria: 1. Mandibular single rooted teeth diagnosed with symptomatic apical periodontitis. 2. The preoperative pain score more than 54 on the Heft Parker Visual Analogue Pain Scale 3. Teeth with mature apices. 4. Healthy patients with age 18 and above.

Exclusion criteria

Exclusion criteria: 1. Patients with systemic disorders, patients taking steroids, pregnant patients. 2. Patients with a history of allergy to any of the components of local anesthesia, chlorhexidine, EDTA, or sodium hypochlorite. 3. Patients with a history of treatment with antibiotics within 1 month prior to the study. 4. Teeth with previous RCT. 5. Severely damaged or mutilated teeth. 6. Patients not willing to participate in the study and did not sign the consent form.

Primary

MeasureTime frame
To evaluate and compare the impact of PHMB on post-endodontic pain in patients with pulpal necrosis and symptomatic apical periodontitis. Timepoint: 0hr, 6hr, 12hr, 24hr, 48hr, 72hr and 7th day

Secondary

MeasureTime frame
To evaluate and compare the efficacy of PHMB with calcium hydroxide in influencing inflammatory markers and post-treatment pain. To investigate the correlation between the inflammatory mediator levels of Substance P and MMP 9 and the degree of pain relief experienced by patients. Timepoint: 0hr, 7th day
