Inclusion criteria
Inclusion criteria: 1. Adult patients aged 18 years and above diagnosed with advanced cancer (stage III or IV). 2. Able to provide informed consent in English, Hindi or Marathi or have a legally authorized representative. 3. Expected to have a life expectancy of at least 3 months. 4. Willing to participate in ACP discussions and complete assessments. 5. Caregiver available to participate, if applicable.
Exclusion criteria
Exclusion criteria: 1. Cognitive impairment that prevents meaningful communication or decision-making. 2. Severe psychiatric disorders (e.g., active psychosis) interfering with participation. 3. Participation in another conflicting intervention trial. 4. Unstable medical conditions requiring immediate intensive care. 5. Patients who decline ACP conversations or follow-up participation.
