Health Condition 1: E11- Type 2 diabetes mellitus

Interventions

Intervention1: CUSTOMIZED NURSING STRATEGIES: Barriers and enablers will be identified for each patient. Customized nursing strategies to overcome barriers will be applied. total duration of intervent
every 2- 3months, during their visit, routine blood test, including FBS and postprandial blood sugar
diet counselling given. The participants in the usual care group will be provided with booklet after the end line assessment.

Eligibility

Inclusion criteria

Inclusion criteria: ? Age = 18 years, gender– both male and female ? Patients who are diagnosed with type 2 diabetes mellitus for the past one year or longer ? HbA1c level of > 8% ? Patients who can understand Tamil or English

Exclusion criteria

Exclusion criteria: 1. Pregnant women and lactating mothers 2. Patient with significant visual & hearing impairment 3. Patients diagnosed with psychiatric illness
