Interventions

Intervention1: meropenem dose adjustment: patients in Group 2 will receive the standard meropenem 2g every 8 hours for 48 hours, with dose adjustment made only after 48 hours based on their creatinine

Eligibility

Inclusion criteria

Inclusion criteria: All adult inpatients prescribed with Meropenem for sepsis with acute kidney injury with no known chronic kidney disease and documented renal impairment based on creatinine clearance values Patients who are treated with a standard dose of meropenem 2g every 8 hours (prolonged infusion over 3 hours) Patients who are willing to provide informed consent.

Exclusion criteria

Exclusion criteria: Outpatients antibiotic therapy Patients below 18 years of age Pregnant and lactating women Patients with chronic kidney disease
