Inclusion criteria

Inclusion criteria: Patients admitted in inpatient units and receiving the high risk medication

Exclusion criteria

Exclusion criteria: 1 Patients admitted in any location and receiving high risk medication except wards 2 Patients prescribed with high risk medication on OPD basis
