Eligibility

Inclusion criteria

Inclusion criteria: - Patients with a diagnosis of breast, ovarian, colorectal or gastric cancer who have completed curative treatment (surgery, chemotherapy, radiotherapy as applicable and planed) within last 12 months of the screening visit - Disease free at the time of recruitment - ECOG performance score 0-2 - Patient or primary care giver are fluent in speaking and reading either Tamil or English - Are ready to use and familiar with one of the proposed mode of tele-consultation (phone call, video call, email or whatsapp text) - willing for remote follow up and study participation confirmed by signing a written

Exclusion criteria: - participation in another clinical trial - concurrent medical illness requiring more frequent hospital follow up

Primary outcome measure: (measured at 6 months, on remote follow up) -HRQoL measured by the global health subscale of the EORTC QLQ-C30 at 6 months -SCoRe-CS at 6 months Timepoint: 18 months
