Inclusion criteria: Patients prescribed for high risk medication during hospitalization at IIH for treatment under :- surgical ward onco ward Executive ward FICU Private room ward Medical ward

Exclusion criteria

Exclusion criteria: Pateint below 18 HIV Patient & Leprosy Death cases Medico legal cases

Occurrence of high risk policy at each phase of medication use cycleTimepoint: Data would be collected at baseline at Pharmacy, and Rechecked at Nursing station after 3 Hours. All data to be compiled and analysed after 12 weeks
