Eligibility

Inclusion criteria

Inclusion criteria: The target population includes adults aged 18 to 65 years who are critically ill and receiving care in the intensive care unit (ICU). This group comprises patients intubated through a tracheostomy (ICD-Z93) or orally intubated (ICD-Z99.1). Eligible patients must meet specific respiratory criteria, including a positive end-expiratory pressure (PEEP) of less than 10 cm H2O, a fraction of inspired oxygen (FiO2) less than 0.6, and a respiratory rate below 25 breaths per minute. Additionally, patients should demonstrate the ability to initiate spontaneous breaths on a mechanical ventilator, exhibit signs of inspiratory muscle weakness, and respond to verbal commands. The population also includes patients in the post-extubation or post-decannulation phases of care.

Exclusion criteria

Exclusion criteria: The exclusion criteria for the study include patients with specific infections such as human immunodeficiency virus (HIV) (ICD-B20) and hepatitis C virus (HCV) (ICD-B18.2). Other medical conditions that exclude participation are pneumothorax (ICD-J93.9), rib fractures (ICD-S22), and recent thoracic surgery (ICD-J95.1). Patients with severe trauma, severe neurological conditions, or cervical fractures below the C3 level (ICD-S12.7) are also excluded. Additionally, individuals with existing speech disorders, such as aphasia (ICD-R47.01), dysarthria (ICD-R47.1), or apraxia (ICD-R48.2), and those with impaired cognition (ICD-G31.8) do not qualify. Patients diagnosed with COVID-19 (ICD-U07.1) are likewise excluded.
