Patients having heart surgery received a nerve block in their chest with a medicine called ropivacaine, sometimes combined with another medicine called dexmedetomidine, to help manage pain after surgery.

Eligibility

Inclusion criteria

Inclusion criteria: 1.Patient giving consent 2.Age between 18-60years 3.ASA II & II 4.Patients undergoing elective cardiac surgery.(MVR,ASD)

Exclusion criteria

Exclusion criteria: Patients not giving consent. Patients with history of allergy to study drugs(ropivacaine,dexmedetomidine). Patients having abnormal coagulation profile Pre operative haemodynamic instability.(Congestive heart failure ,uncontrolled AF). Patients with moderate to severe PAH,LVEF less than 40%.

Primary

MeasureTime frame
1.Pain scores (VAS two hourly) in first 24 hours after extubation . 2.Total fentanyl(0.5mics/kg) consumption in first 24 hours. Timepoint: 1.Pain scores (VAS two hourly) in first 24 hours after extubation . 2.Total fentanyl(0.5mics/kg) consumption in first 24 hours.

Secondary

MeasureTime frame
1.Time of first rescue analgesia. 2.Length of icu stay. 3.Duration of mechanical ventilation after surgery. 4.Hemodynamic variables two hourly (MAP and heart rate). 5.Drug related side effects(delirium and sedation). Timepoint: Hemodynamic variables 2hourly (MAP and Heart rate)
