Permanent molar tooth with extremely deep caries presenting with symptomatic irreversible pulpitis: spontaneous pain, heightened or lingering response to cold or electric pulp testing, nocturnal pain. 3. Negative response to percussion and palpation. 4. Normal periapical structures (Periapical Index Score 1).

Exclusion criteria

Exclusion criteria: 1. Deep carious teeth 2. Tooth exhibiting negative response to pulp testing. 3. Presence of a sinus tract or swelling. 4. No pulpal exposure after non-selective (complete) caries removal. 5. Exposed pulp tissue appeared necrotic (absence of bleeding) or partially necrotic (insufficient bleeding). 6. Uncontrolled pulpal hemorrhage after the procedure lasting more than 10 min. 7. History of analgesic intake 3 days prior to the day of commencement of procedure.

Design outcomes

Primary

MeasureTime frame
CLINICAL SUCCESS – will include absence of spontaneous pain or discomfort, absence of any signs and sypmptoms, normal soft tissues around the tooth with no evidence of swelling or sinus tract, and negative response to axial palpation and percussion tests. Timepoint: Follow up at 6 months and 12 months

Secondary

MeasureTime frame
RADIOGRAPIC SUCCESS- will include no increase in periapical index (PAI) score, no evidence of extra-radicular or intra-radicular pathosis, and absence of internal or external resorption.Timepoint: Follow up at 6 months and 12 months
