Eligibility

Inclusion criteria

Inclusion criteria: 1) Patients who are diagnosed with hypertension and are on medical management 2) Patients above 18 years of age 3) Patients who are lost to follow- up for the treatment 4) Physicians, Nurses, Medical Social workers, Allied health professionals

Exclusion criteria

Exclusion criteria: 1) Patients who are not consenting to participate in the study 2) Administrative staffs
