Inclusion criteria
Inclusion criteria: critically ill patients who are mechanically ventilated for more than 48 hours will be screeened for the study. patients who have been initiated on spontaneous breathing enrolled for the study. patients who do not tolerate the sponataneous breathing trial will be excluded from the study
Exclusion criteria
Exclusion criteria: Failure to obtain consent neuromuscular disorder pneumothorax cervical spine injuries patients with blunt trauma to chest and abdomen patients with massive ascites /raised intra-abdominal pressure patients who have undergone abdominal or thoraxic surgeries patients who have cuff leak of less than 110 ml
